Description

Over the past decade the number of behavioral health providers working in primary care settings has increased exponentially, reflecting heightened recognition of the benefits of integrated care. This workshop provides an overview of the primary care behavioral health (PCBH) model of integration, a population health, team-based approach for improving the care of primary care patients. Provider- and systems-level processes essential for successful integration are highlighted. Case-based examples illustrate strategies for addressing the complex biopsychosocial needs of patients with chronic medical conditions frequently seen in primary care settings.  Original webinar date: October 14, 2020.
