From Siloed to Integrated Care: Practical Tools and Insights for Effective Interprofessional Teams

Description

This intermediate workshop presents evidence-based practice tools and models of integrated care. Participants increase awareness of interprofessional collaborative competencies and application to three care models that vary in setting, level of integration, population served, and stage of implementation. Key interdisciplinary processes, as well as barriers and facilitators of effective teamwork are reviewed. Practical tools and strategies that align with interprofessional collaborative competencies to support effective team development and sustainability are provided. Original webinar date: October 13, 2020

Julija Stelmokas, PsyD

Julija Stelmokas, PsyD, Ann Arbor VA Healthcare System, MI; Dr. Stelmokas is a board-certified clinical neuropsychologist at the Ann Arbor VA (AAVA) Health Care System, Clinical Assistant Professor at Michigan Medicine Department of Psychiatry, and a Research Scientist at the AAVA Geriatric, Research Education and Clinical Center. She earned her Psy.D. from Pacific University and completed internship and post-doctoral training in clinical neuropsychology at the Ann Arbor VA/University of Michigan Consortium, and an Advanced Geriatrics Fellowship through the AAVA GRECC. Her clinical interests include geriatric neuropsychology and integrative care. 

Bethany E. Grix, PhD

Bethany E. Grix, PhD, William S. Middleton Veterans Memorial Hospital, Madison, WI. Dr. Grix is a clinical health psychologist, staff educator, and Program Coordinator for the Whole Health Interdisciplinary Team at the Madison VA. She earned her degree at the Illinois Institute of Technology and completed her internship and post-doctoral training at the Ann Arbor VA/University of Michigan Consortium. Her clinical interests include chronic disease self-management, mindfulness & values-based interventions, and generating culture change in healthcare delivery.

Dr. Elissa Patterson

Dr. Elissa Patterson is a Clinical Assistant Professor of Psychiatry at the University of Michigan where she serves on the Adult Consultation Liaison Service at University Hospital in Ann Arbor. She earned a Ph.D. in biopsychology from the University of Chicago and Re-specialization certification in Clinical Psychology from William James College in Massachusetts. She specializes in addressing behavioral determinants of health in medical/surgical patients. Dr. Patterson is the 2020 past-president of the Michigan Psychological Association.
